Today these images demonstrate the challenges the clinician has to correctly diagnose an Achilles tendon injury without the use of diagnostic ultrasound. 🩻 Here you can see there is not just one pathology (abnormality), but several! Retrocalcaneal bursitis (RCB) is the next patholog Retrocalcaneal bursitis (RCB) is the next pathology/injury that can occur in the Achilles tendon region. Bursa are present around all joints, but we only visualise bursa when they are inflamed. In these images we can see an RCB with a thickened border and some simple fluid. Also the Achilles tendon has some thickening consistent with some Achilles tendinopathy. Diagnostic ultrasound is excellent for detailing the extent of this injury. With this level of detail we can then decide if this injury is treatable with conservative management i.e. physiotherapy, or injection, or if it needs a surgical opinion. In this weeks post on Achilles tendons we have a t In this weeks post on Achilles tendons we have a thickened mid portion Achilles tendon measuring 11mm, with associated inflammation on power doppler consistent with mid portion Achilles tendinopathy (MPAT). In addition, the tendon has multiple white (hyperechoic) spots, which may represent cholesterol deposits or intrasubstance calcification (hydroxyapatite deposition disease). Using diagnostic ultrasound can give the patient a very detailed examination of the integrity of their injury.
